Why You're Not Losing Weight on Semaglutide

What the science says about variable response — and why it's not your fault

You started semaglutide with real hope. You'd heard about the clinical trials, seen the results in the news, maybe watched a friend drop thirty pounds with ease. So when the scale barely moves — or stalls entirely after a modest start — the disappointment can feel personal. Like your body is broken. Like you're doing something wrong.

You're not.

Semaglutide is a powerful, evidence-based medication — but it doesn't work the same way in everyone. Understanding why requires looking at the full picture: the biology of treatment response, and the many factors that shape whether you're a strong responder, a modest responder, or someone who barely responds at all.

 

What the Data Actually Shows

Average Weight Loss from Clinical Trials

In the landmark STEP 1 trial — the largest phase 3 trial of semaglutide for obesity — participants on 2.4 mg weekly (Wegovy) lost an average of 14.9% of their body weight over 68 weeks, compared to 2.4% in the placebo group. That's a meaningful result. But averages can be misleading.

An average of ~15% body weight loss means some people lost 5% — and some lost 25% or more. Where you land on that curve is shaped by biology, not willpower.

The Wide Range of Response

The distribution of outcomes in semaglutide trials is substantial:

•       Roughly 1 in 3 participants loses less than 5% of body weight — considered a minimal response

•       About 1 in 3 loses between 5–15% — a meaningful but moderate response

•       About 1 in 3 loses more than 15%, with a significant subset exceeding 20%

This variability isn't random. It's biological. And researchers are now actively studying what drives it.

 

Why Your Response May Be Different

Genetics: Your Body's Blueprint

Pharmacogenomics — the study of how genes affect drug response — is an emerging but important field in obesity medicine. Several genetic factors appear to influence semaglutide response:

•       Certain polymorphisms in the GLP-1 receptor gene are associated with a blunted response to GLP-1 receptor agonists. If your receptors are less sensitive to stimulation, you may need higher doses — or a different mechanism altogether.GLP-1 receptor gene variants (GLP1R):

•       Genes associated with obesity risk and energy regulation also appear to modulate treatment response.FTO and MC4R variants:

•       Emerging research suggests the microbiome influences GLP-1 receptor agonist efficacy, possibly through effects on appetite signaling and gut motility.Gut microbiome composition:

Genetic testing for medication response in obesity is not yet standard clinical practice — but it's an active area of research, and it's one reason we don't treat every patient identically.

Hormonal Factors

Hormones other than GLP-1 play a major role in weight regulation, and their status can significantly affect your results on semaglutide:

Thyroid Function

Even subclinical hypothyroidism — a mildly elevated TSH within the "normal" range — can blunt metabolic rate and impair weight loss. Optimal thyroid function matters, not just "within range" thyroid function. This is especially relevant for patients with Hashimoto's thyroiditis, where inflammation can persist even when labs look acceptable.

Insulin Resistance

Significant insulin resistance — common in PCOS, type 2 diabetes, and metabolic syndrome — can create a hormonal environment that works against fat loss. GLP-1 agonists improve insulin sensitivity, but if IR is severe, addressing it directly may be necessary to fully unlock the medication's potential.

Cortisol and Chronic Stress

Cortisol is a weight loss antagonist. Chronically elevated cortisol — from psychosocial stress, poor sleep, or underlying conditions — promotes visceral fat accumulation, drives hunger, and counteracts the appetite suppression that makes GLP-1 medications effective. If you're under significant stress or chronically sleep-deprived, your medication is working against a powerful hormonal headwind.

Sex Hormones

Perimenopause and menopause represent a major inflection point for weight regulation. The loss of estrogen shifts fat distribution toward visceral fat, raises insulin resistance, and increases appetite — all of which can blunt GLP-1 medication response. Women in this transition may need a more comprehensive hormonal approach to get optimal results from weight loss medications.

In men, low testosterone has similar effects: increased fat mass, reduced lean muscle, and insulin resistance that makes weight loss harder regardless of medication.

Sleep: More Pharmacological Than You Think

Poor sleep — defined as less than 7 hours of quality sleep per night — is independently associated with weight gain and medication resistance. Sleep deprivation elevates ghrelin (the hunger hormone) and suppresses leptin (the satiety hormone), directly counteracting semaglutide's appetite-suppressing effects.

Semaglutide reduces appetite — but it can't fully override the neurohormonal cascade that sleep deprivation triggers. If you're not sleeping, you're not giving your medication a fair chance.

Dosing: Are You at the Right Level?

Semaglutide follows a titration protocol — doses increase gradually from 0.25 mg to the target dose of 2.4 mg (Wegovy) or up to 1 mg (Ozempic). Several dosing-related issues can limit results:

Insufficient titration:

•       Some patients are maintained at lower doses due to side effects or prescriber conservatism. Lower doses produce less weight loss — the dose-response relationship is real and clinically meaningful.

Too-fast titration:

•       Rushing dose escalation leads to nausea, vomiting, and often discontinuation before reaching a therapeutic level.

Stopping below the target:

•       Staying at 1 mg when 2.4 mg is tolerated and needed leaves significant efficacy on the table.

Optimizing your dose — at the right pace, to the highest tolerated level — is one of the most impactful clinical levers we have.

Nutrition and Muscle Mass

Semaglutide reduces appetite, but it does not eliminate the need for nutritional structure. Patients who continue high-calorie, low-protein diets often see limited results — not because the medication isn't working, but because adequate protein intake is essential for fat loss that doesn't come at the expense of muscle.

Adequate protein (targeting 1.0–1.2 g/kg of ideal body weight daily) and resistance training are important complements to GLP-1 therapy. Without them, a disproportionate share of weight lost may come from muscle — which worsens metabolic rate and long-term maintenance.
